Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options

At Troy Family Dental, we take most PPO’s and all regular dental insurance plans. As forms of payment, we are happy to accept cash, check, Visa, MasterCard, and Discover. For patients who need them, we also partner with CareCredit to provide low- and no-interest dental financing plans. These plans are designed with your budget in mind, so that you can receive the dental care you need without hurting your pocketbook.

"Dr. Lindahl and his staff are so amazing to work with! You can tell they care about you and your experience while there, and are always looking for ways to make your visit better. From personal experience, I needed a lot of work done and didn't like going to the dentist ( which added to my problem) and working with Dr. Lindahl over the years has shown me going to the dentist isn't always scary, and you just need to find a really really great one to take away your worries. I now travel 40 minutes to see him and it's worth every second of the drive. I couldn't recommend a better office and Dentist for your dental needs! They are the BEST!!!"

"Very little waiting time in the front before being called back to get my teeth cleaned. My hygienist Vicki was very friendly, professional and did a great job. I've seen both Dr. Lindel and Dr. Oge and both are very good, caring dentist. I appreciate that they don't push any treatment on you. They recommend it but the choice is yours to make.
